This smartphone app, WA-HSEQ, is developed by Mellora AS in Norway and funded by the SHIP Grant Program (http://www.lni.wa.gov/
safety/GrantsPartnerships/SHIP/). The app can be used to report any safety related issues and has reporting templates for Good Observation, Near Miss and Accidents.
